Enter the catalogue numbers, one at a time – click on the description in the drop down box – fill in the quantity -- the form will do the rest. If there are any quantity discounts we will apply them when the actual invoice is created. Please keep in mind we have temporarily reduced our minimum order requirement from $20 to $10 due to COVID-19.
Fill in the rest of the form completely -- review your order -- click the submit button. For security purposes we will call you for your credit card information once we receive your order or email you with further billing instructions.

C. Click on "Download (Send Letter)" at the bottom of the form to receive the form to your email only (Order will not be submitted to Microform Models, Inc.). Name the file – save it as a PDF -- email it as an attachment to: microformmodels@msn.com. You may need Acrobat Reader to open the PDF.

NOTE: If you choose the PayPal option, be sure to include the email address you want us to send the PayPal RFP [Request for Payment] notice to. Shipments will not be released until the PayPal RFP has cleared.

Our normal business hours are 8:30 AM to 4:30 PM- EST. Monday through Friday but we do check our email on weekends.
